Download this Information Request Form and save it as an Excel file
Complete the first two tabs on the form: #1 Company Information and #2 Employee Information
Email the completed form to partnerships@numien.com

We will then process your grant application with the Texas Workforce Commission and work with you to schedule your staff's training.
